To report health problems directly to the FDA that you generally have with most/all LED lights or LED screens or to report health problems with specific LED devices, use the Accidental Radiation Occurrence (ARO) Report form 3649, which is in section 6 of this webpage:

The FDA welcomes reports from people both inside and outside the United States.
I recommend direct reporting to the FDA, if possible, to make your voice heard directly. You can basically say "see attached" to a lot of the form questions and then attach your story. You don't need to have technical info about the lights/screens that are hurting you, but if you do, you can send that too.
